OHIO DEPARTMENT OF HEALTH 3RD GRADE SCREENING PROGRAM

The Ohio Department of Health has rescheduled a screening program that was to have taken place at BV and Little Hocking schools on February 18th. The new date is April 1st.  The sceening will consist of a dental exam and measurements of height and weight.  All information will be confidential. Permission forms were passed out in early February for the 3rd grade students to take home.  If you would like your 3rd grader to participate
and you have not already done so, please complete the form and return it to your child's teacher.  Also, if you need a form contact your child's teacher.
